AHRQ Training Program
Application information and Forms

Who may apply:
US citizens or Permanent Residents (have their "Green Card") who are:
- predoctoral fellows who are enrolled in or have been accepted to a PhD program at the following institutions, or
- postdoctoral fellows who are employed or will be employed by the following institutions:
How to apply:
- Choose two mentors: one from informatics or human factors engineering, and one from biomedicine
- Prepare a mentoring plan that clearly describes how the two mentors will collaborate to give the trainee training in both of their disciplines
- Prepare a curriculum plan showing how the trainee will meet the AHRQ Training Program curriculum requirements
- Meet with the Institutional Training Representative for required approval of their proposed project
- Complete required application materials.
Graduate and Postdoctoral Fellowship Application This application form cannot be saved; it must be completed and submitted at one time.
- both predocs and postdocs should include a curriculum plan as part of their mentoring plan (see curriculum requirements for more information)
- your mentoring plan can be identical to that used in the mentor recommendation forms of you primary and secondary mentor
- do not use any Greek letters; write out the name (e.g. alpha, beta), as the system turns Greek text into an unreadable symbol
- keep a separate copy of the text portion of your application; you will not receive a final copy after submission, and a printout will not show all of the text in the text fields
Mentor Recommendation Form Both the primary and secondary mentors must fill out this form; they may use the identical mentoring plan as the applicant.
- this form cannot be saved; it must be completed and submitted at one time;

- the secondary mentor does not need to complete the financial and departmental contact information at the bottom; just type something meaningless into this required field
- both mentors must be members of the AHRQ Training Faculty
- membership in the Keck Center does not automatically make one a member of the AHRQ training faculty; you must apply for membership (See FAQ #8)
Application requirements

- Online Mentor Recommendation (link above) from your primary mentor
- Online Mentor Recommendation (link above) from your secondary mentor
- 2 Letters of Recommendation (letterhead optional) from people other than your mentors. They should email the letters directly to Amy Jackson
- Current resume (Word or PDF format) outlining your academic history and professional work experience: email to Amy Jackson
- Proof of US citizenship (birth certificate or passport) or Permanent Residency ("Green Card"): email a clear PDF scan to Amy Jackson or mail a clear photocopy to the address below.
- Transcripts: official copies preferred, but unofficial copies accepted. If the transcript is in a foreign language, you must submit an English translation. Email Amy Jackson the following as PDFs:

- Undergraduate transcript
- Master's degree transcript (if you have a Master's)
- Current institution's transcript, if you are a predoc
- PhD diploma showing the date you received your PhD
- If you do not have access to a scanner, mail the transcripts to:
